So why not a plea of Guilty.

personally, no. All the same, there may be some genuine medical explanation for why drugs and drink somehow had different effects than previously. The nation is full of drug users who don't go around killing people.
I guess the lawyer thought they'd nothing to lose by giving it a try. But if this works because it didn't happen on previous occasions then to be consistent anyone who tries something for the first time and it goes wrong could claim much the same. Maybe it'll be taken into account when they reach the sentencing part.
