How Do I Make This Shower Work?

I live in a small flat and have an ensuite. The spare/main bathroom has this shower over the bath. I cannot see how to get the shower to work.

There are no switches on the wall and no "knob" that I can see, to direct the water up to the shower head.

Any thoughts?

Thanks. I always forget about the Img, bit. Any thoughts?
There should be a lever/knob on the mixer tap.

If you look, there isn't a knob.
Sorry to be flippant but when I saw your question I thought it was a heartfelt remark from the Tory hatchet man Ian Duncan Smith.
Where the pipe goes into the middle of the unit, does that not lift up?
Have you tried pulling the taps up?
I think I may have seen one like this - the bottom of the hose had a collar that you lifted once the water was flowing through the bath filler outlet.
Is that a knob at the front that you pull out or push in?
Surely one of the knobs directs water into the bath or the showerhead
I'm of a similar view to Dave, I'm sure I've also encountered a shower where you pulled up a collar fitting at the base of the hose.
Please show tinypic closeup of bath/shower connection
Pretty sure that with the water running you lift up the collar around the base of the shower hose, water should then come out of the shower head, if you reduce the flow too much or when you turn off the taps the collar should drop back down and the flow return through the taps.

I feel I'm going to get my hair wet.
The wider collar round the end of the pipe definitely lifts up and down but may be stiff to start with.
