I am stuck though on 2D Composer left a trfile (6)

And i cannot see what the clue is or how 42 fits in. I have the Moniker elsewhere

42a is the 3rd word in the title of a best-selling series I don't intend to read, 1d 2 words begins it and 21a ends it

What I don't get is what to highlight - can it be the 3 letter name of a character?

Quinol, yes, the three-letter name, character. The others all have both names in the grid.
