blue badge application form...what is on it?

(i have posted this as an answer to someone else, but would like a proper answer)
i have hypermobility and i am thinking i will apply and let them decide if i am worthy... just hope on the assessment day i am in a bad way so they can see it.....
does anyone know what the form involves? i would like the opportunity to explain what happens to me... and how it affects me, and that it would be somewhat preventative, because a lot of the time i look perfectly fine...
i am sure these people know the medical situation, but i woul dlike to include proof and links to sites etc to explain further....
i have good days and bad days...walking for even a small distance or having to carry things can put me in serious pain and then i am in pain for days - and each time i am permanently ruining my spine and hips...plus i have asthma which adds to it, so sometimes i feel i would need a badge, other days i am ok
the main reason i havent pushed for it so far is because i dont want to become too sedentiary...i want to keep mobile as its important for the condition... so i would likely only use the badge occasionally, when im really bad or i have to carry heavy things...
one of my biggest fears at the moment is that my furture will be in a wheelchair, (i am only 39) so i want to try to do what i can to protect and prevent that...

I filled out one of these for my mum a few months ago. There are questions on there about your mobility and you have the chance to explain what you can and cannot do mobility wise. You are asked to list any medical conditions, ongoing treatment ect. Personally, I wouldn`t bother giving links to anything. Just give the information they ask for. The form then goes to your GP with an addressed envelope and the GP`s surgery sends the form on to the council.
I have the form in front of me (for my mother's application)
Section A: Details and two recent photos and a £2 fee in Cornwall
Section B: Eligibility Criteria - Blind/Disability Allowance
Section C: For Others, Assessed Eligibility - more notes than anything else
Section D: Driver/Passeneger Status and Vehicle Reg. Plus details on the registered health pro for a possible reference, in my mother's case, her community psychiatric nurse for Alzheimers.

it doesn't matter if it's a 'bad day' for you on the assessment day. you have to answer all the questions, on the form and in person, as if it is a bad day, the worst day for you, because that's when you need the blue badge.
most blue badge holders have good days and bad days, but it's the worst day that counts, the days when you're in pain, can't walk, etc.
don't feel guilty (which is what a lot of applicants do) because when you're having a bad day, the badge could make such a difference to your life.

Obtain a blue badge application form,fill it in with the best of your knowledge and then ask your g.p. if you are disabled enough to qualify,if he/she accepts that you are then your g.p. will inform your local authority who in turn will ask you to call into there office where you will hopefully be provided with your badge , It will cost approx £2.
