Yes. They swell just as they do in a horse a few days before the birth is due.

Y would a cow have 4 teets when it births 1-2 calves?

Each teat on an udder has its own milk gland. Large animals need a lot of milk. One large milk gland is too hard to suck dry - it has to do with tissue pressure. Four small milk glands are more energy efficient than one large one.
