I don't know - but it was mentioned in the Wikipedia entry for 'blog' that AB-Ed linked to in his err.... blog .

disambiguation just means differentiation, or removing of ambiguity. Wikipedia has lots of it: if you just enter, say, Parkinson in its search prompt it will (probably) take you to a disambiguation page where it asks you if you want the disease or the chat-show host and gives you links to each one.
