I thought it was population (P), intervention (I), comparison (C) and outcome(s) (O)

You are quite right so if the researchers question was "effective transition from secondary care to preferred place of care" in others words impoving bed blocking and delayed discharges from hospital to home...with package of social care.......what would you put for the P the I and the O....im goin round in circles
P is the problem....so what is actually the problem? eg Is there a shortage of support in the community? Are people being declared ready to leave hospital too soon? Are families reluctant to have a reli back or engage with the placement process? Does Social services have a funding gap? Is the discharge process in hospital fit for purpose? Because the discharge delay is not actually the problem, its a symptom of the problem(s).
Once you have the problem then you can begin to look at what interventions would change the outcomes.
Outcomes A more accurate estimate of discharge date? better engagement with the family?
Sorry I can’t help with the population or comparison....population might be how many bad blockers a hospital had at any one time sorted by age, social circs and diagnosis. comparison might be stats from another more successful hospitl?

oh I forgot, the other kind of interventions you might wish to consider are those designed to prevent admission in the first place...so more medical support in the community, faster access to intensive domestic support in the home, better turn around in A and E and so on.

This was a big part of my job when I worked in the NHS....we were struggling with the problem around 20 years ago and still struggling when I retired nearly 10 years ago.
