Help With Flexible Working Letter!

I am currently on Mat Leave and need to hand in my letter regarding my return to work in a few weeks.
I before my Mat Leave I worked full time (35hrs) over 4.5 days, however would like to change my hours slightly to 4 days, Mon-Thurs, however still do the 35hrs.
On mentioning it whilst visiting work, my boss has indicated that this would not be possible and that I would be required over my original 4.5 days, as due to lots of changes within our company, she feels that she needs me there.
I have read the Gov literature regarding applying for Flex Working however I am not that hot on letter writing and would like to submit a good letter to ask for this change.
Does anyone know if there is anywhere where examples of letters are which I can read?
