How to find a book without knowing title or author

Many years ago I read a novel about a woman locked in a room listening to the goings on around her.She thinks she is in some kind of home or psychiatric hospital, not sure, but she thinks doctors are trying to kill her. She was actually in hospital in a coma but you dont get to find this out until the end of the book. I think the title was something connected to a round room or a round tower.Anybody know what it's called, would love to read it again.
