I have a friend who has a slight codeine addiction. She can take a break when necessary, but she woke up one morning with a headache and a vibrating buzzing noise in her ears. She knows the codeine can affect the blood vessels in her head, but she wonders if the fact she takes more than two in any one go, could it cause one of the vessels to burst and cause her to have a stroke? If not what damage could it cause if she doesn't control it?

For what reason does your friend take codeine?
How old is your friend and has she any underlying medical conditions?

I cannot convince myself that the symptoms that your friend describes, has anything to do with codeine.

The symptoms of headache and buzzing in the ears occur in many conditions of which stroke and "burst blood vessels are an example, albeit uncommon.

I would advice your friend that if this happens again, to see her GP.
