Pain in my hands and elbow

I have always had pain in my hands for years now (when carrying heavy items or things that cut into my hands ie heavy carrier bags) and just delt with it really.
About two months ago the pain became unbearable in my elbow, fingers (thumbs are fine) and one finger has a hard lump which is very painfull. the pain i experience is like general acheing, to toothache level and stabbing pains in my fingers and this swollen finger. They also become very cold. if i lift anything ie a hardback book my elbow aches badly.
I have had blood tests for athritus and physio (no trapped nerve) also had xray for swollen finger waiting for result) i have been taking indometacin 50mgs four times a day for the past two weeks which has removed the permanent ache in the fingers but no change to elbow and swollen finger. i still cant grip things properly. i'm 36, generally active and i eat well, not overwieght etc.
Has anybody got any ideas to what this could be, as i dont seem to be getting anywhere with the doctors and i'm at the end of my teather.
