3 hours with a heavy weight round your neck and your left arm extended (I'm a bass player) doesn't help much, so I always take a Ponstan about an hour before a gig, and another if I need it before bed. Don't think that heat patches have much therapeutic value, but cold is a definite non-no. I rub my shoulder with Olbas oil, and make sure that I keep my neck and shoulder warm.

I suffer from a stiff left colar bone and top of my back, due to sitting at a desk for 8 hrs a day - Have tried all sorts of patches, gels, sprays etc. Currently using Tiger Balm (Red) - seems to relax the stiffness and it feels hot and cold at the same time (smells of cinnamon). May be worth a try for you.
