61 Year Old Male Is Having Pain When Urinating. What Could It Be?

He has been told it is not a UTI after his doctor sent a week's course of Nitrofurantoin which he has finished but is no better. Has today taken a blood sample to surgery as requested.
He keeps asking me to find out what it could possible be. I've no idea, have you?

why would he think you could find out i wonder? are you a secret diagnostician? :)
where does it hurt? and when? Is he having any dysuria? going more often than usual?
Possible enlarged prostrate ,
The blood sample result will help ascertain the cause.
Prostatitis or a kidney infection are possible
Bednobs is correct it is important to know if the pain comes on at the onset, during or the end of passing water.....however:

The commonest cause is an infection, but viral which would not have been picked up by urinalysis or resolved by antibiotics so:

Virus infection is the favourite.
Enlarged prostate comes second and a stone in the urethra next.
Other causes.....not common.

He may need a camera up the penis to sort this out (Cystoscopy)

I have read sqad's comments out to him and he's happy and says thank you very much. BTW he says it starts hurting when he starts urinating and carries on until the end, but it's not agony, just very uncomfortable.
Ladybirder, please excuse this veering off topic.
When I was a youngster I seemed to be able to pee like a horse. Now, in my mid 70s, when I have an urgent need to pee, I only produce a relatively small volume. Could this be a sign of prostate problems?

the only person over 70 in this country who does NOT have this is
MEEEE because I had 40gy radiotherapy to my pelvis 25 y ago and my prostate went away ( in short)
I don't have a prostrate PP, and neither does Bednobs.

Right, just had a phone call telling me he saw the doctor again yesterday and it was finger up the bum time and she felt an enlarged lobe on his prostate possibly pressing on his urethra. Medication changed to Selexid. Says he forgot to ask what caused it so I said I'd ask on here. Looks to me as if it's something that comes to some with old age??
