The active bleaching ingredient is called Carbamide Peroxide, and comes in gel form, with varying strengths. Most dentist use 22% or 35% strengths, and ask you to apply the gel to a plastic teeth tray which has been previously moulded to your teeth. The results are superb.
Most of the shop bought dental whitening products will contain carbamide peroxide, but in a strength so weak that the bleaching effect on the teeth is minimal.
I know of three friends of mine who have all bought carbamide peroxide 22% gel bleaching kits from some ebayer seller, and I think that they're all happy with the results they've got. I dont know who the seller is, but I imagine that if you were to run a search under 'carbamide peroxide', you'd get something.

If your talking about a bleaching system, for example a gel place inside a guard fitted to the teeth. Then you can get very good results. The kind you buy from boots will be less effective than from a dentist for two main reasons, 1)the concentration of the bleach is lower than that available at the dentist 2)the mouth guard will not fit your teeth accurately.
Problems associated with any form of bleach in a mouth guard system are possible sensitivity of your teeth and gums, and the fact that over time they will fade, unless you top up the bleaching regurlarly. Usually once a month at least.
Whitening toothpastes are entirely different to bleaching systems, they are not generally recommend by dentists since they work purely by abrasion. Whilst they will remove surface staining they will also remove surface enamel ultimately leading to more shine through of the underlying dentine making your teeth appear darker! not whiter.
