not sure how u get rid of it but to stop them going yellow apply a clear base coat before the coloured one and that should do the trick.

you can file over the surface of your nail, (what you would do to take out ridges) that should get it away if its just discoloured from a dark shade of nail vanish. also there are nail vanish removers that contain whiteners which are good but they work over time rather then instant. but yeah prevention is always better, make sure you put base coat under reds and browns to save yellowing!

your nails actually go yellow due to the lack of oxygen to them when you're wearing nail polish. so a base coat won't make any difference!! If you wear nail polish all the time, try to maybe wear it for a week then leave it off for a few days to give your nails a rest. As for getting rid off it, I think it should fade if you don't re-polish them. Try buffing them too before you apply the polish, this brings the blood to the surface under the nail and makes it stronger.
Rub lemon juice on the nail surface regularly - go easy on the buffing though!
Spend a good couple of hours with a 4 way buffer, I used to always wear red and my nails were horrendouse, but them a joined a Tae Kwon Do club and had to have them clear, after spending ages buffing them they look like babies and I haven't wore polish since.
