Changing GP

I've recently changed address so will need to find a new doctor in my area to register with. How do I go about doing this? Do I go into the clinic with my NHS card, ID and home bill and they do the rest?

When i moved I just phoned the new clinic and told them I wanted to register, they then made me an appointment with a doctor and i brought my ID and medical card with me which they then sent off and altered
I recently moved and changed doctor and just phoned up my local GP. Went to see the nurse for a brief run-down of my previous medical history. Took my passport as ID but didn't have my NHS card or number, which wasn't a problem as I just gave them my last GPs details and everything was transfered for me. V easy!
