Take a look at the Burns site there's all sorts of information on there and they will also send you a free sample to see if your pup likes it. Good luck.
What did the breeder feed them on/recommend?
Might be an idea to get in touch with the breeder and see if any of the other pups are having problems.
Are you also feeding any milk based feeds?
Could it be any sort of treat that he is reacting to?
Pups eat lots of other things that are not out of the food bowl so you have to make sure he is not picking something else up that could be causing the trouble (eg cat food if you have a cat, anything in your garden when you let him out for a wee etc.).
Also what does your vet suggest?

How old was he when you got him? 8-9 weeks?
Three different foods in 3 weeks.....you can't expect his stomach to settle! Each time you try a new food you have to give it time for the dog to get used to it.
That said....for a nearing 'giant breed' use a low protein [under 20/21%] adult food from about 16 weeks on. This will give you a nice slow steady growth....or refer to your breeder

If he is only 12 weeks and he has had a bad tummy for 5 weeks he can only have been 7 weeks when you got him. You should have stuck to whatever the breeder was feeding (they should have given you a supply) but even a change of environment can upset a puppy. Are you feeding any puppy porridge/cereal or goats milk? He should be on four meals a day, usually two meaty/complete and two milky meals for breakfast and supper.
While James Welbeloved is a good food I am afraid the others are very low quality and I would put him on a large breed puppy food from either Eukanuba or one of the other better brands such as Purina, Royal Canin but I would go back to the breeder first and see what they feed.
Boiled rice and chicken alone will not be giving him enough goodness, vitamins and protein to help his bones grow strong enough to carry his adult body, and he will end up gangly and weak.
