having my wife/children added to the deeds of my house.

Without using a solicitor, I want to have the deeds of my house changed so the ownership of the property becomes a 4 way ownership (myself,my wife,and my 2 children) instead of just in my name alone as it is at present.
I reside in dudley, west midlands,can anyone please advise me where? if possible,I may get the necessary paperwork to carry out this task and how to go about it.

What would happen if one dies? Do the remaining three inherit, or can it be left to someone else?
If one of the parties become bankrupt, will their share of the house be at risk? Can they use their share as security against a loan?
Can anyone sell their share? If the children marry, what happens?
Who is responsible for maintenance, bills and so on?
This is the sort of thing that keeps solicitors busy for years trying to sort out if it's not drawn up properly to start with.
That's without possible tax implications.

Deeds have long been done away with. It is the Registration at the Land Registry which counts now. There is no reason why you should not do as you wish, but first you have to decide whether you wish to be (a) joint tenants, which is based on survivorship, or (b) tenants in common, which provides shares to do with as you wish, or (c)some form of contract. That decided, if you have a mortgage then you must put your proposal to your lender, and, if they agree (and they do not have to) they will instruct you what to do under the heading of "Transfer of Equity". If no mortgage then you can DIY - simply phone your local District Land Registry Office and they will guide you through the process.
