If the property is mortgaged it will be classed as a transfer of equity and your wife will have to go through a lending application to ensure she can maintain the monthly payments be it by herself or with a guarantor present. The mortgage provider will take care of the legal ramblings and may charge a fee - you would need to speak with them. If you own the house with no mortgage I would contact a solicitor for legal advice, or wherever you have you deeds stored and also contact Land registry as they may be able to point you in the right direction.

If there's no mortgage you can DIY - contact the land Registry for the forms and fees.
