I'm thinking of reverting back to my maiden name to help repair my credit score. Does anyone know how I go about it? And do i need to provide proof of my divorce?

And it is illegal to change your name for the purpose of fraud.
However, you are legally allowed to call yourself whatever you like. It is very simple to revert to your maiden name:
"To get all her documents and records changed, she will usually be required to produce her decree absolute certificate and her marriage certificate (a marriage certificate shows a woman's name before marriage). If you do not have either of these documents to hand, you may find it more convenient to execute a change of name by Deed Poll"

i dont understand how changing your name will affect your credit score - you will still be the same person with the sam e debts or lack thereof. When you apply for credit they ask you for all previous names you have been known as don't they? so unless you are consiering committing fraud to obtain credit i dont see how it will make a difference
What may be useful for you to do is get a copy of your credit reports from experian, equifax and callcredit and make sure that you are not associated with your exhusband.
If there are any associations, then simply file a "Notice of Disassocaition" - easy to do and instructions on how to do it will be given with your credit files.
