I have always wondered if the U. S.state lotteries are carefully monitored/oversight for legitimate pick of winners. In other words are they run in an honest way?

Eventually the error came to light; the operator offered to refund anyone who had selected the number during the relevant period the catch was that you had to have your lottery ticket as proof. But at least they were willing to fund some money the moral is hold on to your loosing lottery tickets.
On another occasion, five people won one of the State lotteries with 5 correct numbers out of 6 the catch was that all five people had selected the same 6 numbers. The lottery operator smelt a rat and investigated. Their investigation revealed that the five people had all taken their numbers from a slip out of a Chinese fortune cookie further investigation revealed that the fortune cookie manufacturer used the same 6 lucky lottery numbers for all their fortune cookies.
The fact that the lottery operators are willing to investigate such occurrences, suggests some might be on the level.
