my nephew is like this hes also two years old and he only goes to the toilet every couple of days he has been known to smell quite bad too not long before he goes.only occasionally does it seem to hurt him and then my sister gives him some fresh orange juice diluted down a bit  with water which seems to help him.

thanks for that info.we do give her prunes & prune juice

but neither has much effect.

The daughter of a friend of mine had this problem until she was about 2 or 3 years old and my friend worried so much about her that she even burst into tears.  However, it did seem to sort itself out eventually.  I think they gave her a fig syrup or mixture (on prescription), but my feeling is that it sorted itself out in time and the prescriptions were more to ease my friend's anxiety than anything else.
Our 3 year old son, Spud - has shown these symptoms since he was around 2.  He only occasionally complains of a sore tummy, but usually just before he passes a stool.  This happens generally only every 3-4 days, but when he does perform, he passes a single log that would bring tears to my eyes.  However, it doesnt seem to bother him.  If there seems to be an undue delay, we find a bit of fresh orange juice usually does the trick.  Our GP says its all quite normal at this age.
My youngest son is like this too.  He's isn't crazy on fruit or veg, he does like strawberries & dried fruit.  Also kiwi fruit is quite good at keeping things moving.  You could try adding mashed veg to mashed potatoes or if she likes spag bol "hiding" vegetables in the sauce.
Try hiding linseeds (from good health shops) in her food, say wheetibix or yoghurt. Will work a miracle! Promise!

I don't know if this helps but my 3 year old nephew had exactly the same problem when he came to live with us.  We tried prunes, dried fruit etc with little, if any, effect.

While discussing it with the guy running the health food shop he said he had the same problem when he was younger & it turned out he was allergic to cows' milk (not lactose intolerant, that's slightly different).  We started to give our nephew soya milk (flavoured at first - he really loved getting his special milk-shake) & within days there was a great improvement.  When we finally cut out all milk from his diet (if you look into this you will be astounded at how many products have milk in them...) the problem stopped entirely.

We were told that he may grow out of it so this Xmas we gradually gave him products containing milk & now he can drink cows' milk again as well as go to the loo without any problems.  No idea if he just grew out of the problem or if he was allergic and cutting milk out of his diet completely & then reintroducing it allowed him to build up a resistance.  Might be worth trying soya milk for a while though?
