It is a difficult situation I know. My best friends sister has mental health problems and lives about 200 miles away from any of her family, so they often don't know what's going on until after the event. However her ex-husband is in the area so he is often on the scene and helps to look after the children, so at least there is someone local to her who keeps an eye on her. If there is absolutely no-one you know in the Birkenhead area you can contact for information (her family, friends) then all I can really think of is that you contact either social services (she may well have a social worker) or her GP. I know that the GP cannot pass information on to yourself, but they could maybe take your details so that if there is a major problem you could be contacted. Probably likewise with a social worker.
Maybe you could spend some time in the area (Liverpool and Chester are both very close and are great places to visit. There are also some lovely places on the Wirral such as Port Sunlight Village), let her know that you will be in the area and make a point a seeing both her and your Grandson a few times. If you visit their home you may be lucky enough to meet some of her friends or neighbours and possibly (depending on them and how they view the situation) be able to leave your phone number with them.
Good luck.
