not sure who you would go to, but when you got the death certificate did you not discuss your concern with the person who would then have taken it further?

It was an other family member who got the certificate and did question cause but was to upset to challange

maybe get in touch with the hospital who issued it or the doctor who issued it or maybe your local health authority
