Mortgage and debt - seriously worried

Hi,
I took a mortgage on with my partner in May 2004 for a flat at �195k. Now she wishes to leave me to go and live abroad with her family, which is not something I'd been expecting. Everything is amicable, but I'm concerned about being able to stay in the flat. Whilst I can afford the mortgage myself, I will not be able to make all my other debt/credit card repayments - it's just too much.
I'm looking at taking a consolidation loan secured on the proprty for �34k to clear ALL my debts (this will reduce my motnhly debt repayments by three quarters) - any advice? My partner is willing to do this in both our names (I don't think my credit rating is high enough to do this on my own) and will do whatever necessary to make sure I don't lose the flat. There is likely very little or no equity in the flat having been in there such a short time.
On a similar note, how do I get the property transferred in my name only? I can't see the lender - Halifax - taking my salary as being enough to support the mortgage on my own (even though with the loan, I'll be able to manage things fine).
I'm frantic with worry - can anybody advise me on a good (and cheap) solution to my problems?
