Togo, if this is genuine, the Doctor may have known for sometime but its only recently that the OP has accepted that he or she has a problem.

Ergo, a visit to the GP who can refer to a community Dietitian would be the best first step, surely?

This person may have sought help whilst really low and will pop back and see the answers.
The dietician whilst a good idea very rarely has the insight in to what a morbidly obese person feels like and how low you can sink when it comes to eating.
