Happy Birthday Traditions

When and/or where did the tradition originate of having birthday cakes, of singing 'Happy Birthday' whilst presenting the birthday person with a birthday cake, and of adorning the cake with the appropriate number of candles, and of asking the birthday person to make a wish before blowing out said candles? Anyone any ideas? Thanks!
