This was one of my favourite all time games, I bought a japanese imported Megadrive when it first came out in 88/89 with this game (Still got it, Japanese manuals are much nicer than th UK ones). Learn the enemy patterns, kick their arses, use Gilius the Dwarf as he is the better fighter (more range). I mastered it to a point where I rarely got hit.
As for the Death adder, save your potions, pick on the giant and his two soldiers, make sure they are out the way first, then drop the two skeletons (Use your magic if about to be surrounded) because you don't need them chopping your back up when you're trying to avoid Deathy's axe. Use your special moves often (Attack+Jump together) then you can face him one on one, get him in the corner and take him apart.
If you get him on his arse, quickly double tap to run + jump + attack, if timed right it will do considerable damage. Don't get too far away from him either or he'll send pain along the ground towards you.

One final note, it pays to play in two player mode, if somebody can help you out it's a breeze. I remember playing my mates and the first battle was to see who could select the dwarf before anyone else, on game start we would both move to the right and bash the buttons as quickly as humanly possible to get the dwarf. fun.
