Remember that a catflap does not necessarily have to be low down. If you don't mind your cat using a windowsill, for example, this might open up other possibilities, provided you also provide some assistance to the cat on the outside of the house (eg a platform to replace the ground).
in american they have a frame with a cat flap in it that can be inserted in your patio door frame and your door pushed into it. how you secure your door after im unsure about, my dads one had a bar on the other door that came down into a holder on the open door to hold it shut (sorry finding it hard to explain) maybe its worth phoneing some double glazing comapnies to see if they can provide or make these. ill get a link to american ones to show you what i mean but they dont have the serurity bar so maybe if you read them they make say how to secure your door. http://radiofence.com/cat_doors_patio.htm
