What to ask a builder

I'm having my house extended for the first time and I've never dealt with builders before. What are the top questions to ask them when getting them in to quote?

Ask them how deep they will be building the foundations? What insurances he has - and how much for?
Payment terms?
What other jobs he has done?
What qualifications does his company subscribe to (NHBC, CORGI, FENSA) whatever (he will probly have t on his headed paper.

Who is designing the extention? Who is arranging teh buiding control? Who is going to contact the council for permission etc etc....
