They're old but in one piece and currently have sand brushed into any gaps but I'm wondering if I can remove the sand and brush in Postcrete as a no-fuss, more solid filler, just mist with some water and the job will be done.

The trouble with concrete, Dougie, is the chippings in it. They're only small in Postcrete, but they will tend to stick out as you try to brush a smooth finish. Ready-Mixed mortar would be easier (sand & cement.)

Get some "Joint-It".
It sets rock hard and doesn't crack as easily as traditional mortar. You brush it in dry, so you don't even need to mix it with water, etc - dead easy to do.
I repointed my top patio about 10 years ago and it still hasn't cracked.
