See a GP for a diagnosis, but it certainly sounds like Osgood Schlatters. It was very painful when he was younger and still gives him trouble on occasions and his knees use to 'give way'.
Osgood Schlatters is not a dangerous disease and is reasonably common in older children and teenagers. It's not a 'disease' as such. It is caused by irregular growth spurts causing muscle to tear from the bone. It does improve as you get older. If it is Osgood Schlatters, your GP will tell you how to cope with it and what type of exercise to avoid, e.g. jumping should be avoided.
