Template for letter advising employer of pregnancy

I have been using the Direct.Gov website for advise on maternity pay/ leave issues recently and a couple of weeks ago i noticed that there was a template you could use for advisior your employer that you intend to take maternity leave etc.
I have gone to the website today and can't seem to find the letter or what section it came under - does anyone else know?

Thanks both for your replies. The only other thing I was wondering is, should I indicate now that i am only looking to return part time (im currently full time) or is that not needed at the moment?
Ultimately this may affect my return date - for example i work mon-friday currently,and If I returned on the same basis my first day back would be a Monday but if i choose to return part time and work only 3 days, my return date may be a Wednesday if i chose to work last 3 days of a week...
Also, what happens with holiday that I earn whilst on leave - coould i potentially add this to the end of my leave?

Yes good idea! Don't suppose you know of an online calculator where you can put in your salary etc and it will work out how much tax you will pay on SMP? I know you pay tax but unsure if I will be worse off than £135 a week or better off if i have tax to come back based on the rediculous amount of tax ive paid so far this year!!?
I doubt you'll get back any tax at this stage so early in the tax year.
How much is SMP per week? I doubt there will be much tax to pay on that

Its £135 per week - I wont be claiming it until September so by that point I would have paid tax on 6 months worth of salary assuming I would have been working 12 months. All very complicated!
You won't get tax back but you probably won't pay any tax on your SMP and if you return to work before the end of the tax year you won't pay much tax on those later earnings
I meant you won't get tax back during the tax year. If you have overpaid tax by the end of thetax year you'll get a rebate or your code for 2013/14
Yes do not mention now that you intend to return part time . You can do that later just before you actually come back. You then have the choice , you may want to return full time but with flexible hours for example , the employer has to allow this if it is possible.
and yes, you can add annual leave to the end of mat leave
