the nhs pay system is rubbish. wait for them to cock something up and take 6 months to sort it out. and if the tax office gets involved as well, you're done for. almost everybody i work with has had serious problems with payroll. keep a record of every shift you do and every minute you are late after a shift. claim that back as time owing (or they will suck your life away!). and especially keep a record of any overtime you do and keep your payslips filed in date order. you have to be very organised, as the payroll system isn't. and when you have a problem, phoning up is a nightmare. as response to your op, keep a record that you may have a lump sum of tax paid in oct - if it is over your allowances at the end of the tax year, you will get it back =o)
