Why not create your own modelled on the template you downloaded, using Word, where you can include only the number of boxes you require?
Start from scratch using word x
I use to use excel for my invoices. Takes a bit of tweaking but pretty easy.
The invoices we sent out have our BACS details on them (account and sort code number) so customers can pay direct through the bank if they want to.

Payment due date?

I don't understand what you mean by "brief details of me" - that wouldn't usually go on an invoice, it would have been on your quotation of the original works.

You need to say also if you are VAT registered?
If it's going to look like a letter head for you, why don't you design and include a logo for your business too?

this is a nice fancy one though so i want to adapt it

i have a normal one too

brief details is just a note of the sorts of jobs i do...just like a header
Good heavens, you don't need to download a template for an Invoice. Basically, it just needs to be a letter (or even an email these days) with the relevant details included. Anything fancy is a bit of a waste. It will go to someone doing the Accounts and I doubt if they'll be impressed by a fancy Invoice, in fact they probably wont even notice it.

Keep it businesslike and professional rather than 'fancy'.
I agree with twenty. Just put the info needed. Like s/he said, it'll go straight to the accounts department.
