Info for marriage certificate

I have two queries.

1. If say, the groom, had been married once before, and he was marrying for a second time, what would be put in the 'condition' field of the marriage certificate?

2. Are poth party's mother's names included on the certificate, or just their father's??

Assuming you are talking about England:

1) Widower or divorced / previous mar

2) Just fathers names
