How was it due to lack of staff? What actually happened?
You need to be more specific. Employers and employees both have responsibilities for H&S.
Please clarify what you mean by 'failing to provide sufficient staff' and how this contributed to the accident

his a health care setting and well staff have been informing managment for quite some time about the work load in a manual handeling capicity, an regulation inspection was carried out 6 months ago+ and they reported that it was a requirement not a recomendation that more staff were needed! this has still not happened! never mind staff who were off sick(work related)and on annual leave were not replaced, this meant staff were still carrying out duties with less/little, which they felt was unsafe ie. 2 persons carrying out a task when it is documented that 3 persons is required. a policy also clearly indicates that managment should provide sufficient staff for the health and wellbeing of their employees.Managment are not yet currently seeking employees. i hope this helps clarify :( if not let me know
