PATERNITY PAY QUESTION HELP NEEDED

I have a few questions regarding Ordinary Statutory Paternity Pay ( Ospp )

I'm due at the end of this month and even though my boyfriends employment are aware of my pregnancy but as he's gone through a lot of personal tragedies this year and he hasn't filled in this form yet, is it too late? ( I'm due on the 21st )

Another question i have is that he works on a 4 day on 4 day off basis so if he's allowed 2 weeks off does this mean he has 14 days from the birth including his days off or would this only be work days so he'd have his days off not counted and 14 days would mean 3 shifts and two days?
