Perhaps you have to visit the registry office and get them.
Visit the registry office they will tell you all you need to know
We had the Registrar come to our home to marry us. Just pop into your local office, they will give you forms and discuss what you need to provide.
You need to contact your local registry office at least 16 days before you would like to get married. They need to display the request for 15 days so people can object for some reason.
The will help you with forms etc.
