Disclosure Under Freedom Of Information Act

Does anyone know of a template which I could use to ask for disclosure of info under the above Act?
Alternatively, what details would I need to include when requesting the info?
This is a completely new departure for me.

I coughed a bit before I wrote the first one.
They are straightforward and for freedom of info rather than data protection are very well run by the Information Commissioner - data protection is definitely second best - odd but true.

Dear Sir,

this is a freedom of information request so please answer within the statutory time.

I would like the following

Councillor A mentioned a report in his speech concerning B on the 21st march meeting of the Procrastination Committee.

please send me a copy of the report and details of the five Reds in the bed that he said he had found.

I will accept an URL if the report is somewhere on your site
and I will accept an email answer.

The details of the Reds under the Bed are probably protected
but there is no reason why you should nt ask

Organisation vary in their responses - Dept of Health was the fastest. 48 hrs. Problem organisations have problem data controllers

Oh you send it to the Data Controller of the organisation you are targeting. Private organisations dont have to comply and will tell you.

If you are rebuffed ask for a review and then if that is not to your satisfaction then follow the complaints route at the Info commissioner site.
