Online Form Fillout

Managed to transfer an application form to Acrobat reader to fill it out. online.All was Ok until the next page . It was a questionaire that required a Y N box selection. I couldn’t manage to get the selection to work. Anyone talk me through it Thanks

It might be worth trying a second download - if that doesnt work can you print the last page fill it in then scan it back in if you have that option. Alternatively maybe a friend / family member could help.
