The fact that you signed the form, even with a false name, is proof that you were aware of the disclaimer. Your claim would not succeed.

So how would anyone know that the signature was not yours? If you were the one signing, then it does not matter what name you sign. Don't forget an X is a legitimate signature, although it is unlikely to be your name!
