my father died 3 weeks ago aged 74, when we registered
the death the registrar gave mum a form to fill in to sort out dads pension so she will get some for herself,looks like the letter has gone amiss any ideas or addresses we can write to to get things sorted out as mum is getting stressed out as she is not getting anywhere

sorry to hear of your loss. I also lost my father recently and registered the death, got the certificate and copies (�2.50 each) and the form for the pension.
Why not return to the registrar and ask for another form or go to the DWP and ask them or visit the citizens advice.
You and your mum should`nt be having this stress at such a time.
