Unfair bank charges - fine from FSA

I understand that if bank's do not supply your statements within a certain time frame from the request, they are liable to a fine from the FSA. Is this is true and if so:
1. How long is the time frame?
2. What is the potential fine?
I am researching this for an atircle with a deadline looming so any info greatly appreciated.

Under Section 42 an individual has the right to make a complaint to the Information Commissioners Office.
Section 60 of the DPA currently provides for:
* On summary conviction, a fine not exceeding the statutory maximum (currently �5,000); and
* On conviction on indictment, a fine (which is unlimited).
