Reply to court summons

I've received a court summons for "Failure to give information of driver....". I'm in the process of responding to this document but it asks to ensure that the following is returned.

Driving licence and counterpart
completed "means enquiry form".

However a means form was not enclosed, is it likely that I need to get this from somewhere else, or is the document standard and should only be completed where its sent out?

phone them!...they will send you another one in about..2 weeks...but dont delay..theres a time limit...and it will cost you up to 2k!
On the court summons will be a telephone number.

Phone them first thing in the morning for advice.

I've checked the court summons and there is no number on either, I'll find this information first thing tomorrow and phone them.
