Section 21 Possession Notice - how to word it?

Hi,
We have a tenant in a property who we would like to leave as we want to sell the house. There are no problems or issues between ourselves or the tenant. I believe we need to serve a Section 21 Possession Notice but there doesn't seem to be a prescribed form for this. Nor can I find an example or template.
As this is the first time we have done this and are a little unsure, can anybody in the know suggest how to word the letter?

though I think you may possibly have to register first.
Please remember to get the dates correct on the notice or it will be thrown out. If theyre in a periodic tenancy then, from what I remember, the notice should end on the last day of a rent period giving at least 2 months notice.

flyciderman, the s21 is for exactly that, to terminate the contract giving at least two months notice. If you dont do it correctly the tenants can refuse to leave and you wont be able to get a Court Order to evict without an s21.
If you have problems with a tenant e.g. non payment of rent etc, you would serve an s8 notice.
