It shouldn't be possible.

The United States Visitor and Immigrant Status Indicator Technology (US-VISIT) Program is an integrated, automated entry and exit capability that records the arrival and departure of any person not a citizen or national of the United States. US-VISIT conducts certain terrorist and criminal checks and verifies identities through comparison of biometric identifiers from a variety of systems. The databases checked include the Consolidated Consular Database (CCD) which stores biometric and biographic records of all who apply for visas and logs all refusals. A person attempting to enter the US via the Visa Waiver Program (VWP) with a current visa refusal logged on the CCD should be flagged for immediate deportation.
This is true Kempie, but maybe the questioner means by swimming across the Rio Grande. There are many got into the USA by this and similar routes.
If you can get into Canada it is all to easy to just walk across the border into the USA.
It's not that easy rollo - took us nearly 2.5 hours when we did it from Vancouver. And a further 1.5 hours when we took the train from Torronto.
Thousands of Mexicans do it every week.
I would be surprised if those "thousands of Mexicans" had applied for and been refused a visa.
