Caution placed on property

My current partner is looking to re-mortgage his property and has just been informed his ex girlfriend placed a caution on it some years ago. Although they lived together for many years they were never married. The property is solely in his name as are all the utility bills etc., which were paid by dd from his sole bank account. We cannot understand what claim she has on the house and how she did this without him knowing
