Online Self-Assessment - Pensions data box query

Have 2 occupational pensions, used the provided calculator to enter the totals and tax.
The next box asks for breakdown - payers, amount and tax for each one. PROBLEM - it's an unformatted box (freehand text) but any data entered here yields "ERROR" message.
The associated "?" (help) box merely repeats the instruction to enter the data, no indication as to format, style or method.

Can anyone help with specific instruction/layout please? I'm a newbie at SA,

Don't use anything except numbers or letters. Computer says noooo to many other things. Just write Standard Life 10000 Tax 2000. Then detail the next one. There is no specific format and the box is new. I suspect you could get away with just entering the totals where stated.
