Paternity - Applying to court - Dna testing

Having been in touch with my local county court about paternity testing i was advised to fill in a court C1 form which i did. A week later i receive a letter detailing the judges comments which where - ' The application should be for a Declaration of Parentage under S55A Family Law Act 1986 and commenced by a petition.

I there an actual 'Declaration of Parentage' form available? My local county court seemed to have no knowledge of one and kept referring back to the C1 form(very frustrating)

'Commenced by petition' - is this just the filing of the 'declaration of Parentage' form(if there is one) at court.

For a declaration of parentage application under section 55A FLA 1986 you 'petition' with Form FL 423 supported by a sworn statement.

i am making headway in obtaining the FL423 FORM. Can someone clarify what a 'sworn statement' entails(STIDW?). Is this just an accompanying letter along with the Declaration of parentage form stating why the petition is being made? Does it take on a certain form/wording?
